Bullying Report Form
Reporting bullying or harassing behavior is the first step toward making our school a safe place for everyone. Please report bullying if you experience or witness it in school, on the bus, or at school events. Quaker Valley School Board Policy 231.01 defines bullying as:
“… a single incident or may involve a pattern of abuse over time and involves a student or students being ‘picked on.’ “
Bullying is characterized by the following behaviors:
“Physical intimidation or assault; extortion; oral or written threats; teasing; “put downs;” name calling; threatening
looks; gestures, or actions; cruel rumors; false accusations and social isolation are examples of bullying.”
Use the following form to report bullying. Fill it out online and it will be submitted directly to the guidance office. Reports are held in the strictest of confidence.
